Dişhekimliğinde Alerji: Genel Tanım ve Test Materyalleri

Dişhekimliğinde restoratif amaçlı kullanılan malzemeler, ağız ortamında özellikle epitel ve bağ dokusu gibi canlı dokularla yakın temas halindedirler ve zaman içerisinde organizma ile etkileşime girip alerjik veya toksik reaksiyonlara yol açabilmektedirler. Bu nedenle, bu malzemelerin mekanik ve fiziksel özelliklerinin yanı sıra biyolojik etkilerinin de iyi bilinmesi önemlidir. Ağız ortamında oluşan alerjik reaksiyonların ayırıcı tanısı çoğunlukla güçtür, çünkü oral müköz membran farklı birkaç olaya aynı cevabı verebilmektedir. Buna ek olarak, alerjik bir durumun saptanması da çoğunlukla kolay olmamaktadır. Çoğu dental materyal, farklı özelliklere sahip elementlerden ve moleküllerden oluşmaktadır. Bu nedenle, kullanılan materyallerin içeriklerinde bulunan elementlerin de biyolojik etkilerini bilmemiz önemlidir. Dişhekimliği literatüründe alerjen olduğu belirtilen berilyum, metil metakrilat ve benzoil peroksit gibi maddelerin kullanımında dikkatli olunmalıdır. Bu derlemede, alerjinin genel tanımı, yama testi ve dişhekimliğinde kullanılan restoratif malzemelerin içeriklerinde bulunan element ve moleküllerin alerjen potansiyelleri üzerinde durulmaktadır.

Allergy in Dentistry: General Terms and Testing Materials

Restorative materials used in dentistry are in close contact with the living tissues, namely intraoral epithelium and connective tissue and may react with the organism and cause allergic or toxic reactions in time. Therefore, it is also important to know the biological effects of these materials besides their mechanical and physical properties. Differential diagnosis of intraoral allergic reactions is often difficult since oral mucous membrane may react same against several different events. Moreover, it is not always easy to detect an allergic situation. Most dental materials consist of elements and molecules having different properties. Thus, it is also important to be aware of the biological effects of these elements that are present in these materials. Attention should be paid when using materials such as beryllium, methyl methacrylate and benzoyl peroxide that are deemed allergens in the dental literature. In this review article, general terms of allergy, patch test and allergenic potentials of the elements and molecules that are present in dental restorative materials have been described.
