Estetik Bölgenin İmplantasyon Öncesi Sert ve Yumuşak Doku Ogmentasyonu ile Hazırlanması: Vaka Sunumu

Amaç: Estetik bölgede horizontal ve vertikal yönde kemik kaybı ile birlikte yumuşak doku hacminde de yetersizlik olan bir vakanın dental implant ile tedavi edilebilmesi için yapılan girişimlerin, klinik ve histolojik sonuçlarının gösterilmesi amaçlanmıştır. Gereç ve Yöntem: Sağ üst birinci keser dişini 9 yıl önce travma nedeniyle kaybeden 20 yaşında sistemik sağlıklı bayan hastaya frenektomi işlemini takiben, titanyum membran, kemik grefti ve bağ dokusu grefti ile sert ve yumuşak doku ogmentasyonu yapıldı. İmplant cerrahisi esnasında sert doku biyopsisi alındı ve kişiselleştirilmiş iyileşme başlığı yerleştirildi. Daimi protetik tedavi öncesi tekrar bağ dokusu grefti ile yumuşak doku konturları düzenlendi. Bulgular: Cerrahi girişimler esnasında herhangi bir komplikasyon ile karşılaşılmadı. Yumuşak dokuların şekil ve yapısı doğal dokular ile uyumlu olduğu izlendi. Histolojik değerlendirmede defekt bölgesinde yeni kemik oluşumu olduğu ve bu kemiğin greft partikülleri ile entegre olduğu saptandı. Sonuç: Estetik bölgenin ideal bir biçimde implant ile tedavi edilebilmesinde titanyum membran, kemik grefti ve bağ dokusu greftinin kombine kullanılması, sert ve yumuşak doku ogmentasyonu gerektiren durumlarda hem klinik hem de histolojik olarak başarılı sonuçlar vermektedir.

Implant Site Preparation with Hard and Soft Tissue Augmentation in Esthetic Zone: A Case Report

Objective: The aim of the present report was to demonstrate the clinical and histological outcomes of a case with both hard and soft tissue deficiency in aesthetic area augmented for implant site preparation. Methods: Frenectomy was performed in a 20 year old female patient who lost her upper right central tooth nine years ago due to trauma, followed by hard and soft tissue augmentation procedures using titanium mesh, bone substitutes and connective tissue graft. During dental implant surgery, hard tissue core was harvested and individualized healing abutment was inserted. Prior to definitive prosthetic rehabilitation a connective tissue graft was used to augment the soft tissue contours. Results: Postoperative healing of each surgical intervention was uneventful. The final soft tissue shape and contour was in line to that of natural tissues. Histological findings revealed new bone formation in the defect site, which was integrated with bone graft substitutes. Conclusion: The use of titanium membrane and bone substitutes together with connective tissue graft for implant site augmentation in esthetic zone presenting hard and soft tissue deficiency yield both clinical and histological successful results.
